Peer Support Services (PSS)

Peer Support Services (PSS) offer individuals the opportunity to receive guidance and assistance from someone who has experienced similar challenges. A Peer Support Specialist is a trained individual who has overcome mental health or substance use challenges and can relate to participants on a personal level. This service focuses on:

  • Emotional Support: Providing a safe space for participants to express their thoughts and feelings.
  • Skill Building: Helping individuals develop coping strategies and life skills to manage daily challenges.
  • Mentorship: Offering encouragement and hope through shared experiences and recovery stories.
  • Community Integration: Assisting participants in connecting with resources and building a supportive community around them.

Peer Support empowers individuals to take control of their recovery and fosters independence through mutual respect and understanding.

Individual Support Services (ISS)

Individual Support Services (ISS) at Focused Care Inc. aim to offer participants direct, one-on-one assistance in areas essential to their personal and functional development. These services support participants’ growth, independence, and overall well-being, fostering skills for daily life and improving access to community resources. Our ISS program is particularly beneficial for individuals with diverse support needs, including those with developmental or physical disabilities. 

Life Skills Development

  • Assistance with daily living activities such as meal preparation, personal hygiene, and household management.
  • Support for improving time management, financial literacy, and communication skills.

Social and Community Integration

  • Support for building social skills and establishing meaningful relationships.
  • Assistance in accessing community resources and participation in local events or recreational activities.

 Employment and Education Support

  • Guidance in exploring employment opportunities and assistance in building job-related skills.
  • Help with identifying and enrolling in educational programs, tutoring, or vocational training.

Health and Wellness Support

  • Assistance with managing medical appointments, medication, and other health-related tasks.
  • Promotion of a healthy lifestyle through exercise, diet, and self-care guidance.

 Goal Setting and Achievement

  • Regular goal-setting sessions to identify participantsobjectives and steps to achieve them.
  • Monitoring progress and adapting support as needed to ensure participants meet their personal and developmental milestones.

Therapy

Therapy is a professional service that helps individuals explore and address emotional, psychological, or behavioral issues in a safe and confidential setting. Licensed therapists work with clients to:

  • Identify Problem Areas: Uncover and explore patterns of thought, behavior, or emotions that are causing distress or dysfunction.
  • Develop Coping Strategies: Provide tools and techniques to manage anxiety, depression, trauma, or other mental health conditions.
  • Promote Healing and Growth: Facilitate personal growth by helping clients process past experiences, understand themselves better, and build resilience.
  • Support Mental Health: Therapy is designed to improve overall well-being by promoting positive mental health practices, self-care, and personal insight.

Therapy may include individual sessions, group therapy, family therapy, or specialized modalities like Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), or trauma-informed care, depending on the needs of the participants.

Non- Medical Transportation

Non-medical transportation is essential for seniors, individuals with disabilities, and others who may have limited mobility or access to reliable transportation. The service ensures that participants can maintain their independence and stay engaged in their communities, helping to improve their quality of life.
